In-patient charter annexed to the ministerial circular nbr 95-22 of May 6th, 1995 relating to the patient rights.

    General policy

1. Private clinic is accessible to all. It is adapted to the handicapped people.

2. The health establishments guarantee the quality of treatments, care and reception. They are attentive to pain's relief.

3. Informations given to the patient must be accessible and honest. The patient joins therapeutic choices which relate to him.

4. Medical act can only be provided with patient's consent.

5. A specific consent exists especially for patients taking part in a biomedical research, about donation, use of human body's elements and for detection acts.

6. The patient is free to leave the establishment, unless law restrictions, after being informed about incured risks.

7. There is consideration for the in-patient. Its beliefs are respected. Its intimacy and tranquility must be preserved.

8. The respect of private life is guaranteed to any patient as well as the confidentiality of personal, medical and social information relating to him.

9. The patient can accede to his personnal file particularly to medical informations via a practitioner freely chosen.

10. The patient expresses his observations on care and reception, and can ask for damages.

    Health :

Information concerning your health will be delivered to you during an individual discussion with your practitioner except in case of urgency or impossibility (unconsciousness, coma or mental illness), so your close relations may be informed.

According to the provided information, you take decisions about your health with your practitioner. You can withdraw your consent or reconsider any decision whanever you want.

    Minors  and under supervision people protection (March 4th, 2002 law) :

Information concerning your health are delivered to your legal tutor. The practitioner has to inform you in a appropriate way and has to implicate you in decision-making too.

If you are minor, the practitioner can exempt himself to obtain the consent of your legal tutors if you are expressly against their consultation.

Nevertheless person of age have to be with you, so that practitioner begin care or operation.

The practitioner is able to overstep those authorizations in case of risks with effects on health of the protected person.

    Trustworthy person :

The private clinic proposes to you to nominate a trustworthy person. This person may be consulted if you were unable to express your think and to receive necessary information to do this. This appointment is done in writing in completing the paper at your disposal. This appointment still revocable. If you wish it, your trustworthy person can help you for administration and to attend medical interviews. This person can be a parent, a close relation or your usual doctor.

    Informations relating to in-patient health :

You have right to consult your medical folder directly or by the way of your practitionner. You have to contact clinic manager by written request with notice of delivery.

After receipt, the establishment bind itself to answer you before eight days. If you wish to consult more than 5 years old information, it would be assign within 2 months. Local consultation is free, however you can obtain a copy on your charge.

    Data processing treatment and freedom :

Except your objection, some informations about you, collected during your consultation or your hospitalization, might be the subject of a data-processing recording  for the exclusive use of your doctor, nationa Health Service or mutual insurance.

According to data-processing treatment and freedoms (January 6th, 1978 law), we inform you that you are able to access to your folder, in contacting Management Team. In the same way, personal datas are transmitted to practitioner in charge of medical information, in the strict respect of the medical secrecy. Accroding to the law, you are able to consult or correct those datas in asking your practitioner.

• On the one hand, the questionnaire which is in this booklet and which will enable you to make us share of your remarks and suggestions.

• In addition, you can in the event of problems address a letter to the Director in order to seize the CRUQPEC installation in the establishment.
